However… it seems fishy because they were all between the dates of 2014-02-12 and 2014-06-02. So all in a four month span 9 years ago. Makes me think there was some sort of bug back then :-/ (example).
Ok, I just spot checked some of them and yeah, they're not right. I checked our code and, yep, looks like a bug got fixed on Thu May 29 03:05:04 2014 -0700, which was pushed into production on 2014-05-30 19:27 -0700.
So yeah, it's never happened in our billions of games, and the ones we have are clearly bugs.

It’s certainly possible, since you could design many shuffles that do it, but it’s very unlikely to happen in practice.
