Why does my name come up twice in the score? Very annoying!

It appears you played the game twice (3 times at this point). All the entries have different times. It appears you played the game on 2 different computers, though it's not really easy to tell that, so it might be wrong.
Ether way, we get reports of games repeating from some players. It's one of those things we haven't been able to duplicate ourselves which makes it hard to fix.
How do you get to Green Felt? Do you follow a bookmark/favorite link, or do you search in Google each time?
If you're following a bookmark, you might try refreshing the page when you load it up. Or make sure there isn't a game number in the URL.
